In compliance with the Affordable Care Act (ACA), Finch AI is required to furnish Form 1095-C, which outlines the health insurance coverage we offered to you for the previous year.
Under the Paperwork Burden Reduction Act, the 2025 Form 1095-C is now available upon request. The Forms 1095 provide information about offers of coverage made to full-time employees.
If you would like to receive a copy of your Form 1095-C, you can request the form through one of the following methods:

Requests can be made at any time, and the form will be provided to you by March 2nd, or within 30 days of your request, whichever is later.

If you are an active or former employee with access to Paycor, you may log in to download your Form 1095-C at any time without submitting a request (most recent year 2025 will be made available by March 3rd). Log in to Paycor with your Username and Password.
